Information to Gather Before You Come In

If you choose not to fill out the pre-exam questionnaire, your exam can be more effective if you bring in certain information. A list containing the following information can help to better assess your condition and insure proper, individualized treatment for you.

  • How you use your eyes at work--note the distance, amount of detail, lighting, and time spent at each task.

    Your hobbies and sports activities.

    Any problems with your eyes, such as: reading or developmental problems, or inconsistencies in performance during hobbies or sports.

    All prescription and non-prescription drugs--many medications can affect your eyes.

    Any health problems you have such as high cholesterol, high blood pressure, or diabetes--many general health conditions can affect your eyes.

    Any health problems such as diabetes or heart disease in your immediate family.

     

  • Any past eye problems such as eye injuries, eye diseases, glaucoma, or macular degeneration.
